Application Checklist

Materials Needed for Online Application

For a preview of the information you'll need to provide in the online application, choose the amount of the grant you would like to request:

Grant Limitations

APPLY FOR A GRANT ONLINE

For grant requests for $10,000 or less, you will be asked to include the following information in the online application.

About your organization:

  1. Organization name (and any DBA or AKA name or abbreviations your organization is known by, if applicable)
  2. Organization mailing address
  3. Phone and fax numbers
  4. Web site address (URL)
  5. Organization's e-mail address
  6. Chief executive's name, title, phone, fax and e-mail address
  7. Contact name (if different than the chief executive), title, phone, fax and e-mail address
  8. Organization's mission statement
  9. Percentage of the people served by your organization who are below 80% of the median income (not required for schools. Schools who are applying, see item 10 below)
  10. For a school-based program, percentage of students eligible for the federal free or reduced lunch program (if you are unsure of the percentage, please visit http://www.greatschools.net/*)
  11. Does your organization have any family members of Washington Mutual employees on your staff?If yes, what is their position within your organization? If no, please indicate.
  12. What year did your organization become legally recognized as a 501(c)(3) by the IRS (if applicable)?

About your funding request:

  1. Amount requested
  2. Briefly describe the program or project for which you are seeking support and how it relates to your organizational mission.
  3. Specific program or project goals, objectives, timelines and anticipated impact in the community (if funding is for an event, please provide detailed information on dates, times, number of attendees expected and deadlines).
  4. Please list the primary beneficiaries of this program or project (note: please indicate the one majority benefiting state, county and ethnicity):
  • State
  • County
  • Ethnicity
  • Demographic (80% or under the area median income?) (not required for schools)
  • Schools (percentage of students on free or reduced lunch)

You will be asked to include attachments with the following information:

  1. Names and professional/personal affiliation of board members
  2. Public schools and governmental entities must submit a letter on official letterhead stating that they are governmental entities, providing their nine-digit tax identification number and signed by the senior officer
  3. Tribal organizations must submit a letter on official letterhead stating that they are classified as governmental entities under Section 7701(a)(40) of the Internal Revenue Code and are eligible to receive deductible charitable contributions under Section 7871(a)(1)(A) of the Internal Revenue Code, providing their nine-digit tax identification number and signed by the senior officer (if applicable)
  4. Detailed budget of the program, project or event for which funds are being sought
  5. Invitation to fundraising or other event (if applicable)
  6. Sponsorship options and benefits (if applicable)

Authorization/Certification
By providing the above requested information, the individual submitting this request for funding certifies that they are authorized to represent the organization applying for a grant and that the information contained in this packet of material is accurate. The submitter agrees that if a grant is awarded to the organization: (1) the grant will be used for the purpose outlined in the grant award letter and may not be expended for any other purpose without prior written approval from Washington Mutual; and (2) information about the organization and the grant may be used by Washington Mutual in any published materials.

You will be asked to include attachments with the following information:

  4. If you received a grant from Washington Mutual in the last calendar year, and have not submitted a final report, we require a report detailing how you used the grant money and what you accomplished with the grant (if applicable).
  5. Are you collaborating with any other organization(s) on this proposal? If yes, please provide their names and attach letters of agreement or other documentation from these agencies (if applicable).
  6. Total operating revenue and expense budget for current fiscal year (schools, please provide this information for your school only, not the district information).
  7. List of all sources of revenue for the current and previous fiscal years (list sources and amounts).
  8. List of the top five (5) current foundations and/or corporations from which you have received funding for this program or project.
  9. Detailed budget of the program, project or event for which funds are being sought.
  10. Invitation to fundraising or other event (if applicable).
  11. Sponsorship options and benefits (if applicable).

Grant Limitations
Washington Mutual does not provide grants to individuals or to organizations that:

  • Are not tax-exempt under Section 501(c)(3) of the Internal Revenue Code, governmental entities, including public schools, or tribal organizations that are classified as governmental entities for federal tax purposes
  • Discriminate for any reason, including race, religion, creed, age, sex, sexual orientation or national origin
  • Are religious, unless the program or project to be funded fits into one of the specified focus areas and is for nonreligious purposes
  • Are seeking funding for programs or projects that are outside the areas where Washington Mutual does business
  • Are fiscal agents or fiscal sponsors
